Home hospitalization
Home Hospitalisation is an alternative care option designed for patients who, without requiring a hospital infrastructure, require complex care that cannot be provided by their primary care team. Medical and nursing care is transferred to the patient's home, thus avoiding hospital admission.
How does the service work?
The team at the Home Hospitalisation Unit attends to the requests made on a daily basis and, together with the requesting doctor or service, determines whether the patient should be admitted to the unit.
Home hospitalisation care will be guaranteed 24 hours a day, 365 days a year, through the coordination of this unit with the Emergency services. Every day, the team at the Home Hospitalisation Unit sends Osakidetza a list of patients indicating the treatments they are undergoing. If patients have any problems between 8:00 p.m. and 8:00 a.m., they should contact 112 and indicate that they are part of the Home Hospitalization Program.
Admission to this new Unit is only possible with the prior consent of the patient and their family members or caregivers. Also, provided that the conditions of the home are suitable for the proper care of the patient's illness and that they have a caregiver who lives with them at home, at least for the duration of the hospitalization.
Specialty
HaD is fundamentally focused on four types of patients:
- Those who suffer from acute infectious diseases and require long-term intravenous therapy.
- Chronic patients who are generally multi-pathological: chronic patients (suffering, for example, from heart failure, chronic bronchitis, cirrhosis, complicated diabetes, etc.).
- Patients who have undergone surgery and require post-surgical care.
- Terminally ill patients with palliative treatment.
Patients are cared for by medical and nursing professionals with specific training in this type of care, having access at home to most of the diagnostic and therapeutic techniques that are provided in a hospital.
Centers
The Home Hospitalisation Unit of Asunción Klinika covers the area of Tolosaldea and Andoain, under the agreement with the Department of Health of the Basque Government.
