Hospital

An indicator of hospital care quality that can be used to assess the care process is the prevalence of hospital readmissions, as it reflects the impact of hospital care on a patient's condition after discharge. Therefore, one of the main objectives in the hospital setting is to measure the prevalence of hospital readmissions, identify possible factors influencing readmissions, and predict a possible readmission at the time of patient discharge, in order to prevent potentially avoidable readmissions.

The main objective of the REINGRES project is to develop a comprehensive system for healthcare professionals and healthcare managers. On the one hand, the system will provide healthcare professionals with a comprehensive and streamlined overview for monitoring patients at potential risk of readmission, with all the necessary information available in a comprehensive and visual format. On the other hand, REINGRES will help healthcare managers identify the causes of readmissions and the most vulnerable population, thereby helping to optimize resources.

Reingres is a Hazitek project (File No.: ZL-2020/00332), co-financed by the Department of Economic Development, Sustainability and Environment of the Basque Government and the European Union, through the ERDF, within the framework of the Basque Country Operational Programme ERDF 2014-2020.

Goals

  • To determine the prevalence of readmission in patients.

  • Analyze the possible relationship between different risk factors and readmission

  • Improve patient morbidity and mortality

  • Analyze the economic impact of readmission and economic improvement of healthcare costs in our population

  • To establish, using current knowledge based on scientific evidence, corrective methods so that it is possible to implement the necessary measures to prevent readmissions and optimally manage hospital discharges.
